Event Waiver and Release of Liability

I know and understand that while participating in [volunteering for] the Miles4Miles 1 Mile & 5K Walk and Run I will be using a City of Omaha park where hazards exist, and I am aware of and appreciate the hazards that may result. I voluntarily assume all risks associated with participation in this event, all such risks being known and appreciated by me. I acknowledge and represent that I am medically able and sufficiently trained to participate in an event of this nature. I know that participation carries an inherent risk that accidents may occur during this event, including, but not limited to those caused by physical exertion which could result in serious injury to my person, death or property damage. I accept all and full responsibility for any injury or accident resulting from my participation in this event, including, but not limited to those which may be attributed to weather conditions.

In consideration of your accepting my entry and of being permitted to participate in the Miles4Miles 1 Mile and 5K Walk and Run, I for myself and anyone entitled to act on my behalf, for my heirs and assigns, estate, personal representatives and next of kin hereby waive and release, forever discharge and agree to hold harmless the Archdiocese of Omaha, St. Wenceslaus Catholic Church and St. Wenceslaus Catholic School of the Archdiocese of Omaha, Miles4Miles, the State of Nebraska, Douglas County, Nebraska, the City of Omaha, and all respective members, affiliates, their agents, employees, volunteers, officers, directors, successors and assigns, any and all event sponsors and volunteers and their representatives and successors, from all claims or liabilities of any kind that may arise as a result of my participation in, or that I may cause to others as a result of my participation in this event, even though that liability may arise out of negligence or carelessness on the part of the persons and/or entities named in this waiver.
I grant full permission to any and all of the foregoing to use any photographs, videotapes, website images, motion pictures, recordings, or any other record of these events for any legitimate purpose, including commercial advertising, without monetary or other compensation to me.
