Waiver

I know running/walking in a road race is a potentially hazardous activity. I should not enter unless I am properly trained and in good health. I assume all risks associated with participating in this event. Having read this waiver and knowing these facts and in consideration of your accepting my entry, I, for myself and anyone entitled to act on my behalf, waive and release the race organizers, volunteers, officials, Fort Wayne Trails, its Board of Directors and members, the City of Fort Wayne, Parkview YMCA, Salomon Farm Park, and all sponsors, their representatives and successors, from all claims, damages or liabilities of any kind arising out of my participation in this event; even though that liability may arise out of negligence or carelessness on the part of the persons named in this waiver.

I grant permission to all the organizers/persons of this event to use any photographs, motion picture, or recordings for promotional purposes. Registration is NON-REFUNDABLE due to cancellation for any reason, or any other negligence or carelessness on part of the person named in this waiver.

By signing this registration form, I also accept and agree to the terms of the USA Track and Field waiver of Liability and assumption of risk, viewable on the Fort Wayne Trails website at: fwtrails.org/wp-content/uploads/2021/01/2021USATFWAIVER.pdf
